An individual can be admitted to
the Brentwood for sub acute
rehabilitation or to the
Buckingham
long
term care facility and
can come from the hospital
or the community. When a resident
is
admitted from the hospital, medical and insurance information
is
provided by that hospital.
When a resident is admitted
from home or anywhere else in the
community, a History &
Physical form must be filled out
by the
resident's primary care physician and insurance
information must
be provided by the responsible
party.
Prior to admission into our facility, the individual's medical
and
financial information (i.e.
Medicare,
Medicaid, Managed Care,
other insurances, or private
pay) will be reviewed by the
administrative staff. Once an
individual is accepted, an admissions
application
must be filled
out and the resident will be welcomed
into our facility.
